Description: "Home Is Where The Heart Is" is an all day conference sponsored by the Western New York Alliance for Person-Centered Care (WNYAPCC) that will focus on the "how to's" of initiating culture change in long term care and assisted living facilities. From this seminar attendee's will gain a thorough understanding of the neighborhood concept which is a model providers may use to facilitate person-centered care on individual units; have a better understanding of the eleven updated Federal regulations as it relates to person-centered care, how to develop an "I" plan of care and allow elders to have more involvement in the day to day decisions of their lives thereby creating a life worth living; and understand the ways of turning an institution into a home, employee into a care partner and a patient into an individual.
